If you are a Graduate Nursing Alumni and you need to request a professional liability claims history report for a credentialing process, the graduate and employer will need complete the following:
- Graduate - Complete and sign the PSON Consent to Release Claims History form.
- Employer - Complete the below online Claims History Request Form.
- You will need to attach the completed PSON Consent to Release Claims History form
signed by the graduate permitting the insurance company to share a report with a third
party.
